Food inspections from Oct. 20 to Nov. 5, 2009 | Inspections

food_inspectionArkansas Department of Health investigators completed inspections of  food establishments in Washington County between Oct. 20 and Nov. 5, 2009. Information provided in this article is compiled from those reports.

Inspectors said the following food establishments had critical violations of health standards:

Pho Quyen, 1241 Martin Luther King in Fayetteville
Critical violations: No open cups in food prep area, must have chlorine solution to wipe down food prep area, do not use pesticides labeled for home use, do not store products in ice used for human consumption, keep soap and paper towels at all times at hand wash sink and keep food properly heated and refrigerated

EZ Mart #402, 1950 N. Leverett Ave. in Fayetteville
Critical violations: Need paper towels at handwash sink

Charlie’s Chicken, 1542 Martin Luther King Blvd. in Fayetteville
Critical violations: food needs to be properly refrigerated and heated, do not store de-greaser/sanitizer above food.

Event Group, 2418 N. Gregg in Fayetteville
Critical violations: label all chemical spray bottles

Bypass Mini Mart, 2817 N. Garland in Fayetteville
Critical violations: keep mop sink hose out of mop sink or put flow device back on

El Chico Restaurant and Grill, 3854 Front St. in Fayetteville
Critical violations: plates and tortillas containers require cleaning, hand soap and paper towels required in bar area

Louie’s Grill and Bar, 102 E. Joyce Blvd. in Fayetteville
Critical violations: food employee drinks require lids

Ichiban Hibachi Grill and Sushi, 637 W. Joyce in Fayetteville
Critical violations: Food employee drinks require lids, need a written procedure for sushi rice, must post consumer advisory for sushi in lunch menu

Harps #176, 1189 N. Garland in Fayetteville
Critical violations: Insecticide should be labeled for food service, sanitizer must be properly mixed, dishes must be sanitized

The Perk 2, 3980 Wedington in Fayetteville
Critical violations: label all spray bottles properly

Washington Elementary School, 425 N. Highland Ave. in Fayetteville
Critical violations: keep sanitizer in wipe down buckets, properly mix sanitizer

La Huerta Bar and Grill, 4901 S. Thompson in Springdale
Critical violations: food employees shall change gloves when contaminated (handled raw beef and cracked eggs then touched a cooked quesadilla)

Domino’s Pizza,1065 N. Garland Ave.
Critical violations: label all spray bottles properly

Sonic, 2113 N. Center in Elkins
Critical violations: employee restroom and back kitchen hand wash sinks require paper towels, manager dipping tobacco while preparing drinks

Tim’s Pizza, 2576 W. Sunset Ave.
Critical violations: need paper towels in employees’ bathroom

Inspectors said the following food establishments were inspected and found to have non-critical violations of health standards:

  • Taco Bell, 1828 N. Crossover in Fayetteville
  • Super Foods, 36 Main St. in West Fork
  • Ozark Sunrise Donut Shop, 88 N. Centennial in West Fork
  • EZ Mart #432, 54 Centennial in West Fork
  • Mojito’s Mexican Grill, 100 E. Joyce Blvd. in Fayetteville
  • Shum Berris Donuts, 113 W. Pitman in Prairie Grove
  • Jerry Williams Elementary School, 322 Broyles St. in Farmington
  • North College Super Stop, 357 N. College in Fayetteville
  • Harps 170 Deli and Bakery, 1945 Butterfield Coach Road in Springdale
  • North Mini Mart, 785 North St. in Fayetteville
  • Paschal’s Grocery, 20553 Durham McCord Road in Durham
  • Whistle Stop Senior, Hwy. 16 in Durham
  • The Muse Coffee House and Gallery, 12103 Hazel Valley Road in Durham
  • The Airport Cafe, 802 Airport Road in Springdale
  • Neighborhood News, 4201 N. Shiloh in Fayetteville
  • Boars Nest Express, 4201 N. Shiloah in Fayetteville
  • Market Place Express, 4201 N. Shiloh in Fayetteville
  • Mi Tienda Supermarket, 1505 S. Thompson in Springdale

Inspectors said the following food establishments were inspected and had no violations of health standards:

  • Just Like Home Childcare, 3075 Mission in Fayetteville
  • Happy Hollow Elementary School, 300 S. Ray Ave. in Fayetteville
  • La Despensa Latina, 3138 Old Missouri Road in Springdale
  • J O Kelly Middle School, 1879 E. Robinson in Springdale
  • George Junior High School, 3200 Powell St. in Springdale
  • George Elementary School, 2878 Powell St. in Springdale
  • Southern Ice Distributors #1, 4315 E. Mission in Fayetteville
  • Tropical Snow Portable, 4315 E. Mission in Fayetteville
  • Asbell Elementary School, 1500 Sang in Fayetteville
  • Club Haus Fitness, 612 W. Dickson St. in Fayetteville
  • Elkins Elementary, 476 N. Center in Elkins
  • Dollar General, 2393 N. Center in Elkins
  • Doodlebugs Day Care and Preschool, 1228 Brookhaven in Springdale
  • Village Market, 2103 S. Powell in Springdale
  • Daylight Donuts Express, 2675 W. Sunset in Springdale
  • Lenny’s Sub Shop, 160 E. Joyce in Fayetteville
  • Dippin’ Dots, 4201 N. Shiloh in Fayetteville
  • EZ Mart, 1540 15th St. in Faytteville

Inspectors said the following food establishments were OK to open:

  • Pang Catering Kitchen, 5188 Buterfield Coach in Springdale

Inspectors followed up on the following food establishments to make sure violations were corrected:

  • Ichiban Hibachi and Sushi Grill, 637 W. Joyce in Fayetteville
